mHUB's COVID-19 Policies & Procedures
mHUB remains committed to the fight against COVID-19 and making our community safe. Below you will find updated policies and procedures in our efforts to mitigate the spread of the virus.
Proof of Vaccination Requirement
Effective November 1, 2021, all staff, mHUB members, mentors, instructors, and event guests will be required to present proof of vaccination upon entering mHUB.
The verification process will be:
mHUB members will need to complete the vaccination survey and verify with proof of vaccination. If you have not yet received it, please contact
team@mhubchicago.com
Starting on November 1, 2021, any members who have not completed the survey or who have not shown proof of vaccination will lose access to the building.
All event guests will need to show proof of vaccination at event check-in at mHUB.
Proof of vaccination means that two weeks have passed since the final shot in the immunization regimen.
Vaccines will be verified with state issued ID accompanied by a physical vaccination card, photo of the vaccination card, or official
app tracking from your pharmacy/primary care doctor/government site/etc.
Vaccine Exemptions
Active pregnancy or active breastfeeding only if the provider is requesting an exemption. The exemption lasts only until you're no longer actively pregnant or actively breastfeeding. Pregnancy and breastfeeding are not contraindications for vaccination.
Immunocompromised individuals only with provider request for an exemption and only for those who have recent (within the past 3-6 months) hematopoietic or solid organ transplant, or on active treatment with Rituximab within the past 3-6 months.
Have received COVID-specific monoclonal antibodies in the past 90 days.
Anyone wishing to apply for a medical or religious exemption to mHUB’s vaccination requirement should complete this form.
All members that are exempt from the vaccine will need to show proof of negative PCR test weekly.
Mask Requirements
Masks are required in all areas of mHUB regardless of vaccination status.
Masks can be taken off when seated 6 feet apart from someone if you are consuming food or drinks.
Masks can be taken off in a private office when an individual is alone.
Visitors
Visitor hours are Monday through Friday 8 am – 5 pm.
Visitors are not allowed to work in the prototyping shop. A member or staff can take them through on a tour, show them a piece of equipment, but they cannot stay in the shop to work or support a project that’s being worked on.
Cleanliness & Ventilation
Hand sanitizer is available at stations around the building.
mHUB has state-of-the-art HVAC with controls over the system, which constantly cycles in outside air.
